Su Casa Residential Redesign: 24/7 Substance Use Treatment in NYC

Su Casa is a NYS OASAS Part 820-licensed residential treatment program in Manhattan’s Lower East Side. We provide high-intensity, 24/7 clinical care for adults with Substance Use Disorders (SUD) and co-occurring medical or mental health conditions. As one of the only non-profit organizations in NYC with an integrated Opioid Treatment Program (OTP) on-site, we provide seamless access to Methadone and Buprenorphine in a safe, residential setting.

How Does the Residential Redesign Program Work?

Our program follows a goal-oriented pathway designed by the New York State Office of Addiction Services and Supports (OASAS). Unlike traditional rehab, Su Casa is goal-sensitive, not time-sensitive.

Stabilization

This initial phase focuses on managing withdrawal symptoms and establishing a stable foundation for recovery. Medication-Assisted Treatment (MAT) plays a crucial role in this process, helping clients safely navigate the challenges of detoxification. The stabilization phase typically lasts between 30 and 90 days, with the duration tailored to each individual’s unique needs and progress. This period allows clients to adjust physically and mentally, preparing them for the more intensive rehabilitation work.

Rehabilitation

Once stabilized, clients transition into the rehabilitation phase. This intensive program is designed to equip individuals with the tools and skills necessary to maintain long-term sobriety. The focus is on developing healthy coping mechanisms for triggers, implementing relapse prevention strategies, and cultivating positive social skills. Through a combination of group and individual therapy sessions, clients explore the underlying issues contributing to their addiction and learn how to manage their recovery journey effectively. This phase empowers clients to take ownership of their recovery and build a strong foundation for a healthier future.

Reintegration

The final element of the Residential Redesign Program is reintegration. This phase prepares clients for a successful return to their communities. Recognizing that recovery extends beyond the treatment setting, this phase provides crucial support in areas such as housing, vocational training, job searches, resumé writing, and connecting with essential community resources. As clients gain independence and demonstrate progress, the intensity of treatment gradually decreases to a minimum of 5 hours per week, facilitating a smooth, sustainable transition back into their community. This gradual reduction in support allows clients to maintain their progress while adjusting to independent living.

Residential vs. Outpatient: Which Level of Care Do I Need?

  • Choose Outpatient if: You have a stable home environment and can manage daily triggers during clinic visits.
  • Choose Su Casa (Residential) if: You require 24/7 medical monitoring, have complex medical needs (like HIV/AIDS or chronic illness), or lack a safe, drug-free place to live. Su Casa provides a supportive environment where the peers around you help you work towards a resilient recovery.

Specialized Programs: Pregnant Women and Infants (PWIP)

At Su Casa, we believe in keeping families together. Our Pregnant Women and Infants Program offers a specialized track where mothers can receive high-intensity treatment while living with their newborns.

  • On-site Nursery: A safe, licensed environment for infants.
  • Parenting Support: Specialized counseling for new mothers in recovery.

Integrated Opioid Treatment Program (OTP) in NYC

One of the most critical barriers to recovery is having to leave a facility to visit a methadone clinic. At Su Casa:

  • Medicine is On-Site: We are the only non-profit OASAS 820 Residential Redesign program in New York City with a fully integrated, on-site Opioid Treatment Program. Residents receive Methadone, Buprenorphine (Suboxone), or Vivitrol directly within the safety of our facility.
  • Integrated Medical Staff: Doctors, nurses, and counselors work in the same building to ensure your care is coordinated 24/7.
  • Dual Recovery Focus: We specialize in patients with co-occurring disorders (Dual Diagnosis), ensuring mental health and physical health are treated simultaneously.

Evidence-Based Clinical Modalities at Su Casa

Our residential community acts as the primary vehicle for change. We integrate specific, peer-reviewed therapies into every resident's daily schedule:

  • Individual Counseling: Weekly one-on-one sessions with a Qualified Health Professional to address personal triggers and trauma.
  • Group Therapy: Daily facilitated sessions focusing on relapse prevention, emotional regulation, and social skill development.
  • CBT & DBT Skills: We use Cognitive Behavioral Therapy to restructure thought patterns and Dialectical Behavior Therapy to manage intense emotions.
  • Trauma-Informed Care: All clinical staff are trained in protocols to help residents heal from the trauma that often drives substance use.

Yes. We accept Medicaid, Medicare, and most private insurance. A sliding-fee scale is available for those without insurance to ensure no one is turned away.

A sober house is just a residence. Su Casa is an OASAS 820 Clinical Program with 24/7 medical staff, nurses, and licensed counselors on-site.

Stays are based on individual progress through the three elements, typically ranging from several months to a year.

The primary difference is that the redesigned program now provides comprehensive treatment for all substance use disorders, not just opioid addiction. While our commitment to holistic and individualized care remains, the program structure is now organized around the stabilization, rehabilitation, and reintegration phases to better support clients at different stages of their recovery journey.

No, the program is client centered. Your treatment plan will be tailored to your individual recovery goals and needs. You will work with your treatment team to determine the appropriate phases and duration of your stay. Some individuals may benefit from all three phases, while others may only require support in one or two areas.

The length of intensive residential treatment is determined by several variables that are decided individually between the client and staff. The clients set the goals, and we work with them to accomplish them in a way that leads to a resilient recovery.

Family members can visit by appointment only. Typically, visits are set up by your assigned case manager in conjunction with your treatment plan.

Su Casa recognizes the importance of family involvement and can arrange home visits once you are stabilized in treatment and can demonstrate the ability to manage activities of daily living without the use of drugs and/or alcohol.

As long as an employer allows a leave of absence, a client may be able to work once they are stabilized in treatment and can demonstrate the ability to manage daily living activities without using drugs and/or alcohol. This is decided on an individual basis between the client and staff.

While Su Casa does offer some gender-specific services, most of our residential treatment programs are co-ed.

While this varies somewhat due to layout, most bedrooms are set up in a college dorm fashion with a bed, closet, and dresser.

You will be eligible to have a cell phone thirty days after your arrival for use after program service hours.

You can have a limited amount of money in your possession while in treatment. Please note that you should not bring more than $20 when you arrive for admission.

Vocational and educational opportunities are available. Every resident in our treatment programs will have an assigned vocational counselor who will work with you to develop an individualized plan to meet your needs.

Yes, typically, the starting dose is 30mg. The medical director works closely with each client and will evaluate them based on their individual needs.

While in treatment, you will work closely with your primary counselor to fill out the 2010E form to begin your housing process.

Yes, we do. However, it is best to have paperwork from your previous providers sent in as soon as possible.

To be eligible, you should currently be using or have a history of dependency on substances. If you've also experienced mild mental health issues alongside a substance use disorder, we offer psychiatric services. Additionally, if you're seeking support on behalf of a family member or loved one impacted by addiction, we’re here to help.
